Key Worker Model
Key Worker Model is about more than just connecting you with providers. It’s about helping you understand your NDIS plan, make informed choices, and coordinate the right supports so they work together effectively for you and your family.
As a Social Worker and Early Childhood Practitioner, my approach to Key Worker Model is:
Family-centred and practical
Grounded in capacity building, not dependence
Focused on real-life outcomes, not just paperwork
I support participants and families to:
Understand their NDIS plan and budgets
Identify and engage appropriate providers
Coordinate allied health, education, and community supports
Navigate changes, reviews, and life transitions
Build confidence to manage supports independently over time
My goal is to work with you — building your skills and understanding — so you feel confident managing your plan and know when and how to seek guidance when needed.
With significant changes coming to the NDIS over the next few years, having informed, ethical Key Worker Model now can make a meaningful difference.
If you’d like to know whether Key Worker Model is right for you or your family, feel free to get in touch.
